How do I get access to my medical records?
For you, as a patient, there are processes in place to allow you to review your medical record. Your bedside nurse or nurse manager of your hospital unit can help you with this upon request. After you are discharged or at the end of your care episode, you may request copies of the finalized patient record. Contact Customer Service Reception at 39276866 for more information.
- Complete all fields on the authorization form(s) when requesting the release of your records.
- If you do not know your VITUP Healthcare ID number, leave it blank，we will help you fill in.
- After the form(s) is signed and dated, hand it over to Customer Service Center or mail it to the address indicated.
Typical processing time to request medical records is 5-7 working days, depending on the availability of the medical record.
